Investigators say a July 2005 fire at BP's Texas City refinery could have been prevented.
A report released Sunday by the US Chemical Safety and Hazard Investigation Board said workers installed pipe that was not resistant to high temperatures.
The fire occurred at the same plant where 15 people were killed in a March 2005 explosion
