Pharmacist Emergency Dept Full Time (Job Number: 134442)
Dispenses medications and pharmaceutical supplies prescribed by physicians. Reviews and interprets medication orders to ensure overdoses have not been prescribed and incompatibilities or toxic compounds will not result from combining ingredients. Contacts and confers with physicians regarding any irregularities. Consult patients concerning medication reconciliation, disease state management, and discharge medications. Monitor and provide recommendations for Core Measures including but not limited to glycemic control, anticoagulation protocols, and disease state management protocols. Participate in all trauma and resuscitation codes in the ED, cardiac catheterization lab, and surgical departments. ACLS required. Doctor of Pharmacy, Board Certification, and graduate of ASHP accredited residency preferred. Schedule: 7-on/7-off, 11-hr shifts.
