Self-care management and how it helps keep heart patients out of the ER

Self-care management is crucial for heart failure patients, and now, a program from CHI St. Joseph Health is seeking to help patients do just that.

Guideline Directed Medical Therapy (GDMT) is an intense process of medication management that involves more than filling a prescription at the drug store. However, it also leads to better outcomes for heart failure patients.

"It means they don't end up back in the ER as often," said Wendy Seneff, a nurse practitioner who works with GDMT patients.
